Intradialytic hypotension is associated with dialitic age in chronic hemodialysis patients

Razionale

Intradialytic hypotension (IDH) is common in patients on chronic hemodialysis, but knowledge on determinants is still unclear. The present study aims at evaluating the association between IDH and dialytic age in hemodialysis patients.

Casistica e Metodi

We enrolled 68 patients admitted to the hemodialysis Unit of the Catholic University, Rome. IDH was defined as the symptomatic fall in systolic blood pressure under 100 mmHg. Dialytic age was ascertained using medical records. Linear regression was adopted to assess the adjusted association between IDH and dialytic age.

Risultati

The mean dialytic age was 92 ± 81 months. The presence of IDH was detected in 11/68 (11.2%) patients. The mean number of intradialytic hypotensive events was 0.8 ± 0.2. The main characteristics of participants are depicted in Table 1. There was a positive correlation between intradialytic hypotension and dialytic age (rs=.24; P=.042).

Multivariable analysis

According to linear regression, dialytic age was associated was associated with intradialytic hypotension in the unadjusted model (B=.02; 95% CI=.01-.03; P=.042), after adjusting for age and sex (B=.01; 95% CI=.01-.03; P=.042), as well as in the multivariable model (B=.02; 95% CI=.01-.02; P=.027, Table 2). Also, systolic blood pressure was positively associated with intradialytic hypotension (B=.02; 95% CI=.01-03; P=.031), as well as haemoglobin levels (B=.30; 95% CI=.08-.53; P=.009). Eventually, interdialytic weight gain was positively associated with intradialytic hypotension (B=.21; 95% CI=.04-.38; P=.017).

Conclusioni

Dialytic age is associated with an increased probability of IDH and with increased number of IHD events. Dedicated studies are needed to understand the underlying factors of such an association.
